SC / ST Caste Certificate (Central Proforma)
Scheduled Caste / Scheduled Tribe certificate issued in the prescribed constitutional order format. Does not expire annually.
PwD Disability Certificate from 16 MCC Designated Centers
Disability certificate issued exclusively through the online portal of one of the 16 designated MCC Disability Assessment Boards.
Academic Gap Year Affidavit (Dropper Declaration)
Notarized legal declaration on ₹50 or ₹100 non-judicial stamp paper stating non-enrollment in any other college and non-involvement in illegal activities.
Name Discrepancy / Spelling Mismatch Affidavit
Notarized affidavit on stamp paper declaring that minor spelling variations between Aadhaar Card, 10th Marksheet, and NEET Admit Card refer to the same individual.
Compulsory Rural Service & Discontinuation Bond
Indemnity bond with two solvent sureties guaranteeing completion of compulsory government rural service and payment of penalty if seat is discontinued.
Anti-Ragging Undertaking (Student & Parent)
Dual online affidavit registration on antiragging.in as mandated by NMC Medical College Regulations, submitted with registration reference number.
Transfer Certificate (TC) / School Leaving Certificate
Original Transfer Certificate issued by the Principal of the last attended school or junior college, certifying regular student conduct.

Character & Conduct Certificate
Certificate issued by the head of the institution last attended, confirming exemplary conduct and no disciplinary reprimands.
State Domicile / Permanent Residence Certificate (PRC)
Statutory proof of residential domicile required for claiming 85% State Quota seats as per state medical admission rules.
NRI Sponsorship Letter, Passport & Embassy Certificate
Required for 15% NRI Quota: Sponsor resident visa, valid passport, sworn relationship affidavit, bank statements, and Indian Embassy verification certificate.
Minority Status Certificate (Religious / Linguistic)
Proof of belonging to Christian, Muslim, Jain, Sikh, or Linguistic minority for institutions like CMC Vellore, Hamdard Delhi, SGRR Dehradun, or KMC Manipal.

What happens to my original documents after physical admission?
Admitting medical institutions retain all original certificates (10th, 12th, TC, migration, domicile, category) until university registration is complete (usually 6-12 months). They will issue an official "Original Document Custody Receipt". Always keep at least 10 high-resolution color photocopies and scanned PDFs beforehand.
Why must EWS / OBC-NCL certificates be dated after April 1, 2026?
EWS and OBC Non-Creamy Layer statuses depend on the previous financial year's gross income (FY 2025-2026 ending March 31, 2026). Hence, any certificate issued prior to April 1, 2026 is legally invalid for NEET UG 2026 counselling and results in automatic forfeiture of the reserved category seat.
What if my name spelling differs between Aadhaar and 10th marksheet?
Minor spelling discrepancies, missing middle names, or initials can be accepted at reporting centers only if accompanied by a notarized "Name Variation Affidavit" on ₹50/₹100 non-judicial stamp paper stating both spellings denote the same candidate.
